Assisted Living

Eligibility Requirements

In order to be eligible for Assisted Living, a person must: 

  • Be insured under the Health Insurance Act of Ontario (OHIP) and have a valid OHIP card
  • Be 16 years of age or older
  • Have a permanent physical disability and requires physical assistance with activities of daily living
  • Require personal support services on a 24-hour basis and have care requirements that cannot be met solely on a scheduled visitation basis. The person shall require services in a frequent, urgent and intense manner described as follows:
    • Frequency – individual requires multiple/ intermittent visits throughout the day on a 24-hour / 7-day a week / 365 days per year basis with an average of 3.5 hours of care per day up to a maximum of 6 hours of care per day
    • Urgency – individual warrants prompt response that cannot always be scheduled
    • Intensity – individual’s condition or predicament requires direct personal attention from staff to address needs
  • Be able to direct their own personal support and homemaking services
  • Be medically stable or able to have his/ her medical needs met by professionals in the community (i.e. Home and Community Care Support Services, family physician, etc.)
  • Have services that can be provided in a person’s own home and when required, in their place of employment or place of education where they are pursuing adult education courses to obtain a degree/certificate/diploma
  • Demonstrate a greater need for personal support services over and above homemaking services.
  • Be able to be left alone between visits (i.e. does not require constant supervision)
  • Ensure services can be provided safely in their home; and that the nature of the services and service environment do not pose a serious risk to the person, attendant or organization
  • Reside in Waterloo Region
