Attendant Outreach
Eligibility Requirements
In order to be eligible for Attendant Outreach services, a person must:
- Be insured under the Health Insurance Act of Ontario (OHIP) and have a valid OHIP card
- Be 16 years of age or older
- Have a permanent physical disability and require physical assistance with activities of daily living
- Have service needs that can be met on a pre-scheduled visitation basis
- Be able to direct their own personal support and homemaking services
- Understand their service requirements, what procedures are necessary to meet their service needs, and be able to provide instructions to an attendant on how to carry out these activities and/ or procedures
- Be medically stable or able to have his/ her medical needs met by professionals in the community (i.e. Home and Community Care Support Services, family physician, etc.)
- Have services that can be provided in a person’s own home and when required, in their place of employment or place of education where they are pursuing adult education courses to obtain a degree/certificate/diploma
- Demonstrate a greater need for personal support services over and above homemaking
- Be able to be left alone between visits (i.e. does not require constant supervision)
- Ensure services can be provided safely in their home; and that the nature of the services and service environment do not pose a serious risk to the person, attendant or organization
- Reside in Waterloo Region
